Empowering Communities Through Prevention: The Ernest Foundation's Community Health Awareness Programme
Good health begins with knowledge. When people understand how to prevent illness, recognise early warning signs, and access the right healthcare services, they are better equipped to live healthier and more fulfilling lives.
The Ernest Foundation is proud to work in partnership with Lester Practice to deliver a comprehensive Community Health Awareness Programme, bringing essential health education and free health checks directly into the heart of the community. Supported by the NHS South East London through the London Borough of Southwark, this initiative aims to reduce health inequalities and improve health outcomes for local residents.
Why Community Health Awareness Matters.
Many long-term health conditions such as diabetes, high blood pressure, heart disease, kidney disease, and certain cancers often develop silently, showing few symptoms in their early stages. Unfortunately, many people only seek medical help when these conditions have become more serious.
Through education, early screening, and community engagement, our programme encourages residents to take control of their health before problems arise.
By increasing awareness and promoting early intervention, we hope to reduce preventable illnesses and improve the quality of life across our communities.
What the Programme Offers
The Community Health Awareness Programme provides free health information, practical advice, and access to preventative health services in a welcoming and supportive environment.
Participants can benefit from:
• Free NHS blood pressure checks for adults
• Advice on maintaining healthy blood pressure
• Guidance on healthy lifestyles and disease prevention
• Signposting to appropriate NHS services where necessary
• Opportunities to connect with healthcare professionals and fellow community members
Our sessions are designed to be accessible, informative, and culturally sensitive, ensuring everyone feels comfortable seeking advice about their health.
Health Topics Covered
The programme focuses on some of the most important health issues affecting our communities, including:
• Cancer awareness and screening
• Cervical, breast, and prostate health
• Cardiovascular disease prevention
• Diabetes awareness and management
• Hypertension (high blood pressure)
• Chronic kidney disease
• Child health and immunisation
• Drug and alcohol awareness
• Mental health and emotional wellbeing
• Social prescribing
• Digital health education and access to NHS services
These sessions provide practical information that participants can apply immediately to improve their health and support their families.
Supporting Health Equity
Health inequalities continue to affect many communities, particularly among Black, Asian, Minority Ethnic, and disadvantaged populations. Limited access to health information, delayed diagnosis, and barriers to healthcare can lead to poorer health outcomes.
The Ernest Foundation is committed to addressing these challenges by providing trusted health education within community settings where people feel safe, respected, and understood.
Working alongside healthcare professionals enables us to bridge the gap between local communities and primary healthcare services.
The Importance of Early Detection
Regular health checks can save lives.
A simple blood pressure test can identify hypertension before symptoms appear, reducing the risk of stroke, heart attack, kidney disease, and other serious complications.
Likewise, understanding the importance of cancer screening, diabetes monitoring, and cardiovascular health empowers individuals to make informed decisions and seek timely medical advice.
Our message is simple:
Prevention is always better than cure.
Building Stronger, Healthier Communities
This programme is about more than medical information—it is about creating healthier communities through education, empowerment, and partnership.
By bringing together healthcare professionals, local organisations, and residents, we are fostering a culture where wellbeing becomes everyone's responsibility.
Every conversation, every health check, and every piece of advice shared has the potential to transform lives.
